The Healthy Aging & Memory Clinic

The Healthy Aging and Memory Clinic is comprised of a multidisciplinary team that assess individuals’ overall health as it relates to their cognitive functioning. This can include diagnosing, treatment planning and connecting individuals and their caregivers/loved ones to community resources where appropriate. This appointment helps provide you, your caregiver/loved one, and your Physician/Nurse Practitioner with a deeper understanding of your condition. The clinic’s overall goal is to help patients who have cognitive concerns maintain independent living and their quality of life for as long as possible.

During your appointment you and your loved one/caregiver will meet with our interdisciplinary team; including a Nurse Practitioner, Social Worker, Pharmacist, and representatives from the Alzheimer’s Society and Home and Community Care Support Services.

Please speak with your Family Physician/ Nurse Practitioner if you would like an assessment with the Healthy Aging and Memory Clinic. Once referred our reception team will reach out to you with an appointment date and time and provide you and your family with more information. You are encouraged to bring with you a caregiver/loved one that knows you well. You will have a follow up appointment after with your family physician.
