Since 2006 we have been growing and refining our Family Health Team so that we can care well for our patients. From our Clinic in Clinton our interdisciplinary team provides primary health care to patients and families in Clinton, Bayfield, Blyth, and Goderich area with patients coming from as far as Walkerton, London and Stratford.

Our Mission:

We will provide timely patient centered primary health care through a multidisciplinary team of providers.  This team will partner with the patient, caregivers and community services to improve health outcomes.

Service Excellence | Dimension: Patient-centred

Percent of patients who stated that when they see the doctor or nurse practitioner, they or someone else in the office (always/ often) involve them as much as they want to be in decisions about their care and treatment.

Change Idea #1:

Re-establish patient experience intake and survey process and deploy at all locations.

Method: Identify methods for regular survey intake such as online and by paper, as well as survey periods and methods.

Process Measures: Percent of Primary Care Providers with patient experience surveys deployed.

Target for process measure: Percent of Primary Care Providers with patient experience surveys deployed: 100%

Change Idea #2:

Align and collaborate with Huron Perth primary care organizations on a standard patient experience questionnaire that will both contribute to identifying opportunities to improve primary care within each organization as well as inform the Huron Perth and Area OHT.

Method: The Huron Perth FHT ED Group will approve standard patient experience survey questions and include them as part of their Patient Experience survey process.

Process Measure: Percent of Huron Perth and Area FHT’s including standard patient experience questions.

Target for process measure: Percent of Huron Perth and Area FHT’s including standard patient experience questions: 100%

Change Idea #3:

Establish baseline patient experience data both locally as well as regionally within primary care in the Huron Perth and Area OHT.

Method: Each FHT will establish local baseline data. The FHT ED Group will establish a process for sharing and collating regional patient experience data, as well as a reporting mechanism and dashboard

Process Measures: 1. Percent of primary care organizations in Huron Perth and Area OHT who have established baseline patient experience data and targets for 2024/25. 2. Monitoring and feedback process established by FHT ED Group(YES/NO)

Safe & Effective Care | Dimension: Safe

Percentage of non-palliative patients newly dispensed an opioid prescribed by any provider in the health care system. The source period will be the 6 month period ending Mar 31, 2022 where current performance is 3.70, target is CB. The target justification is: Rates of opioid prescribing have been on a slight downward trend, based on the Primary Care Practice Report data. Our objectives related to opioid prescribing focus on increasing awareness, education and collaboration across our health care system. We continue to monitor our prescribing rates and aim to sustain or reduce current performance.

Change Idea #1:

Identify collaborative opportunities across Huron and Perth for education and training of staff and/or physicians and provide educational opportunities.

Methods: The Huron Perth FHT ED Group will collaborate to identify regional educational opportunities of benefit to our local system. This would include communicating with local stakeholders outside of primary care ( such as surgeons or dentists for example).

Process Measures: Percent of identified target group for education who attends education or training.

Target for process measure: Percent of identified target group for education who attends education or training: 75%

Comments: Thai process measure and target are subject to change based on the identified target group for education and training.

Change Idea #2:

Implement and/or update and reintroduce opioid prescribing and support tools in primary care across Huron Perth.

Methods: The Huron Perth FHT ED Group will collaborate and review available tools or features within electronic medical records used in Huron Perth. Each primary care organization will update and reintroduce tools appropriate within their organization. Where possible tools will be aligned.

Process Measures: Percent of Huron Perth primary care organizations who have updated and reintroduced opioid prescribing and management tools.

Target for process measure: Percent of Huron and Perth primary care organizations who have updated and reintroduced opioid prescribing and management tools: 100%
