LAB Hours

You MUST call to book an appointment.
The LAB is Closed for lunch daily from 12:40 pm to 1:30 pm.

No After Hours Clinic

If you have a True Emergency, please go to the closest Emergency Department. If you are in a life threatening situation call 911.

If you need a non-urgent care outside of clinic hours you can connect with Health811. They have both call, live chat and TTY options available.

If you would like to make an appointment for any of our clinics please call our office or use our online booking below.

In order to book your appointment appropriately, please provide our staff with an accurate reason for your visit. This reason will stay confidential and allows us to ensure you are seeing the appropriate person and receive the appropriate care.

If you would like a referral to any of the internal programs such as diabetes education, our dietitian, memory clinic, our mental health team, and/or any groups we are running, etc., please talk to your health care provider or contact reception.

Patient Rights & Responsibilities

Patient Rights

As a patient of the Clinton Family Health Team & Clinton Physicians, you have the right to:

  • Know the names of the people providing you with care.
  • All calls to our nursing staff are triaged for urgency & depending upon volume/urgency a return call may take 2-3 business days
  • Your forms will be completed as quickly and efficiently as possible. Form length and complexity may extend the required time, please allow 4-6 weeks. Providers require an appointment prior to completing a form
  • Receive care that is respectful, non-discriminatory, considerate and timely.
  • Ask questions if you do not understand any information given to you.
  • Be an active participant in your care and make choices about your treatment, including refusing treatment, and be informed of the health risks of your decision.
  • Complete and up-to-date information about your diagnosis, treatment, and prognosis, in terms you can understand, in order to participate in discussions regarding your care.
  • Be told if something unexpected and significant happens in your care and have any resulting changes to your care discussed with you.
  • Make choices about how your information is collected, used and disclosed by us.
  • Expect that all communications and records pertaining to your care be available only to those directly concerned with your care.
  • Access your health record.
  • Expect that your personal health information will be treated in a confidential manner by all staff and physicians involved in your care./li>
  • Express your concerns about the Clinton Family Health Team / Clinton Physicians or its staff and receive a response.
  • Receive complete information about any research project in which you are invited to participate, with an option to refuse to participate without it affecting your care.
  • Authorizing someone, other than you, to communicate with our staff about your personal health information. You can download the ‘Authorization To Disclose Form” below.

Patient Responsibility

As a patient of the Clinton Family Health Team and Clinton Physicians, we ask that you:

  • Tell us about any concerns you have about the safety and/or quality of your care.
  • Treat our Family Health Team / Clinton Physician staff, physicians, other patients and visitors with consideration and respect.
  • Arrive at appointments and/or classes/workshops/groups promptly or, if they must be canceled or postponed, we ask that you notify us as soon as possible (at least 24 hours in advance).
  • Be open and honest with us about your health history and health care needs (including your past and current illness, allergies, medications and dosages, and family health history).
  • Number of issues per appointment – may need longer appointment time and/or additional appointment to keep clinic running on time
  • Let us know about any changes in your contact information.
  • Please be aware that if you add your first and last name to your phone voicemail we are able to leave more detailed messages about your health. If there is no name attached to your voicemail, you will be asked to return our call to hear the same information.
  • Take an active role in your own care. You can do this by:
    • Being proactive in keeping yourself healthy.
    • Using the advice and information from your health care providers.
    • Asking questions to clarify information.
    • Telling us what is important to you.
