Are you open for drop-Ins?

No. Our location runs by appointment only. We would be happy to see you, but need you to book in with us. You can call 519-482-3000.

How do I cancel my appointment?

To cancel an appointment please call 519-482-3000. We can help reschedule you. Please give us as much notice as you can. Thank you.

Are any of your services not covered by OHIP?

These are called uninsured services and examples are: driver’s license assessment, completion of some medical forms, transfer of records, travel injections, etc. In these instances, payment by cash is required and you will be billed at the time of your appointment. 

What if I don’t have OHIP?

It is the patient’s responsibility to have a valid health card.  Please bring your health card to every visit.  To renew your health card, please visit a Service Ontario Centre in person or Online.

If you do not have a valid health card, there is a charge for appointments at Clinton  Family Health Team.

How do I renew my prescriptions?

We request that medication renewals be managed during your regular appointment with your family doctor whenever possible.  Please bring all your medications to every visit.

For medication refills without an appointment, please contact your pharmacy directly. Please allow 5 – 7  business days for the processing of pharmacy refill requests. A renewal of a narcotic requires an appointment with your prescribing Doctor.

What is a Family Health Team?

A Family Health Team (FHT) is an approach to primary health care that brings together different health care providers to coordinate the highest possible quality of care for you – the patient. Our team consists of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills within the clinic to provide you with the very best and most up-to-date care, when you need it.

Travel vaccinations

Travel vaccine consultation or administration is not offered. Please reach out to a travel vaccination clinic. We recommend you book an appointment at least 3 months in advance of international travel plans to ensure that any and all vaccinations are completed on time. We recommend the following:
