Prerequisites:

Registration:

Please speak to your Family Physician or Nurse Practitioner for a referral to the Mental Health Team.

Once a referral has been completed, a person from the Mental Health Team will reach out to you to complete an Intake. This will involve discussing why you would like counseling, assessing if there are other more appropriate resources in the area that could better meet your needs, and potentially offering strategies if appropriate. If you have benefits through work or an Employee Assistance Program we kindly ask that you utilize these services first before seeking counseling from the Clinton Family Health Team counselors.
